People Using Service

Pattom supports people, of all ages, with a learning disability and/or mental health condition and complex needs.


Pattom support people with Commissioned care packages, funded care through the local county council, Private clients and Young people transitioning from children’s services.


Pattom’s service users include young people going through ‘transition’, generally being people between the ages of 16 and 25 years.

Transition is the period during which these young people, with a learning disability and/or a mental health condition, move from children’s services to adult services within the social care, health care and education sectors.

Pattom understands that the transition from school can be a daunting and distressing time for young people, and their families.

Our support aims to ensure that these young people remain as unaffected as possible by the changes going on around them. There should be a smooth transition of services.

We aim to achieve this by providing consistency and reliability before, during and after the period.
